Admission to Charlene’s Angels Adult Day Center will be open to individuals meeting the admission guidelines without regard to sex, race, creed, or religious belief.
Before placement, the Executive Director will meet with potential participants interested in attending Charlene’s Angels Adult Day Center. The Director will evaluate each participant and determine if needs can be completed at the center.
The ADS Level of Service Assessment Tool will be utilized to determine each participant’s level of service using a point system. Charlene’s Angels Adult Day Center will accept participants meeting Level 1 Service, which cannot exceed 11 points, and Level 2 Service, which cannot exceed 22 points.
To be considered for admission, the following guidelines are followed:
Charlene’s Angels Adult Day center will accept participants who meet the criteria for the Basic Adult Day Service (Level 1 and Level 2) as follows:
a. Monitor and/or supervise all Activities of Daily Living (ADL’s are defined as dressing, bathing, grooming, eating, walking, and toileting)
b. Comprehensive, therapeutic activities
c. Assure health assessment and intermittent monitoring of health status
d. Monitor medication
e. Ability to provide appropriate structure and supervision for those with mild and cognitive impairment
f. Staff to Participant ratio for Level 1 not to exceed 1:8. 1:6 for Level 2
Before admission to our Adult Day Program, the participant/responsible party will provide the Center with in-take documentation and general information, including medical and social history, emergency contact information, and other pertinent information. You may obtain these forms on your tour of our Adult Day Program.
Before services can start for Respite or PAC Services, all intake documentation must be completed and returned to Charlene’s Angels. Please email Alana Swain at aswain@charsangels.com for intake forms.
Management reserves the right to decline admission to any applicant for whom Charlene’s Angels determines, in its sole judgment, that it cannot meet his/her needs.
